Completion of membership application form

  •  Two (2) forms of valid identification
  •  Utility bill/bank statement (no older than one (1) month)
  •  Proof of Employment – Job Letter (no older than one (1) month)
  •  Proof of Income – Pay slip (within 1 month)
  •  Registration fee of $15.00 (TT)  non-refundable 
  • Approval is up to the discretion of the Board which typically meets on the second Tuesday of 
    every month. 
  •  A salary deduction/standing order is to be completed
  • Completed forms must be dropped off at the office during business hours.
