Completion of membership application form
- Two (2) forms of valid identification
- Utility bill/bank statement (no older than one (1) month)
- Proof of Employment – Job Letter (no older than one (1) month)
- Proof of Income – Pay slip (within 1 month)
- Registration fee of $15.00 (TT) non-refundable
Approval is up to the discretion of the Board which typically meets on the second Tuesday of
- A salary deduction/standing order is to be completed
- Completed forms must be dropped off at the office during business hours.
